HPV Year 8 Programme

the NHS Vaccination team are due to visit school on Thursday 9^th June  to administer HPV (Human Papillomavirus) vaccination to all Year 8 pupils. 

Please complete the form to CONSENT or REFUSE the vaccinations by following the link below, which will close midnight on 15^th April 2022

When filling out the form please opt for the YEAR of birth BEFORE the month.
