The Lancashire immunisation team are due to visit school on Thursday 14th March to administer Diphtheria, Tetanus, Polio and Meningitis ACWY vaccinations to Year 9 pupils.    

Please complete the form to CONSENT or REFUSE the vaccinations by following the link below, which will close at midnight on the 1st March.
